Time Off Request Form

ILLNESS OR EMERGENCY ABSENCE: In addition to completing this form, contact your Principal/Supervisor, or their assistant, as early as possible. Please do not leave a voice message--try to speak with someone.

PLANNED ABSENCES:  Must be approved in advance and require AT LEAST 48 hours’ notice.

PERSONAL ABSENCE: Must be approved in advance and require AT LEAST 48 hours' notice. Personal time cannot be taken in August or May.


You are responsible for your tracking your time off.

Please track your time off during the year and compare your notes with your division administrative assistant to be sure your attendance record reflects the days you are not able to be in attendance at school. You can verify your time off on record at www.payrollpartners.com by clicking here.  (You will be required to login to your employee account).

If in excess of allowable days, salary will be adjusted accordingly. Your daily rate will be deducted from your pay if the days used are in excess of your allowed time off.

SECONDARY SUBSTITUTE PROCEDURE:

  1. Complete the sub request (link) and submit (it will go to the principals and Nichole Surratt).
  2. The principals will review your request and either approve or deny it. Then Nichole Surratt and Jonathan Kidd will record your leave/absence request. When you receive an email by one of the principals, it will indicate if your request has been approved.
  3. Once your name has been posted on the calendar, you are responsible for finding a sub for yourself from the approved-sub list (also attached). Be sure to check the calendar to see which subs might already be working on that day/those days.
  4. Once you find a sub, send Nichole an email letting them know who will be subbing for you. The sub’s name will be posted on the calendar for reference on the day of your absence.
  5. It is your responsibility to ensure that the substitute has the details of your lesson plans, extra duties, etc. You can either email them to the sub or leave them on your desk, in your office, etc.
