This form should be submitted and approved by a principal prior to announcing the showing of any multi-media format. All multi-media presentations must have an academic, spiritual or social objective. One you submit your request, you will receive an email of approval or denial within 48 hours. The media must be approved prior to presentation. This includes any downloaded materials from the internet. Unless a written exception is granted, NO MORE than two showings per month are allowed in any given class.
